CEDAR POINT TRIP LIABILITY WAIVER AND RELEASE OF CLAIMS
I, the undersigned participant (or parent/legal guardian of a minor participant), acknowledge that participation in The Threshing Floor Church Cedar Point Trip on August 15, 2026 involves travel and recreational activities that may carry certain risks, including but not limited to injury, illness, accidents, property loss, or other unforeseen circumstances.
I voluntarily choose to participate and assume all risks associated with this trip.
In consideration of being permitted to participate, I hereby release, waive, discharge, and hold harmless The Threshing Floor Church, its pastors, leaders, volunteers, staff, representatives, and affiliates from any and all claims, liabilities, demands, actions, damages, costs, or expenses arising from or related to participation in this event, including transportation to and from Cedar Point.
“A separate registration and waiver form must be completed for EACH individual attending the trip, including members of the same household.”
